These forms are not designed for data entry on a computer.
If you don't have Adobe Acrobat Reader installed on your computer, this software is easily accessible on the Adobe website.

Word and Excel files are designed to be "data entry friendly", but you must have these programs loaded on your computer
in order to edit them. Always click the "Enable Macros" button (if prompted) to give you full functionality.
To open a particular Customer Form file you will need the compatible software for that particular file type and then:
- click on any of the file links below and allow ample time for the file to download and open.
- after the file opens, users can save a copy of the file to a local hard drive if they choose.

To save a form file directly to your hard drive:
- right click on any of the file links below.
- choose "save target as" or "save link as" depending on your web browser.
- choose the location where you want the file stored and click save.
- allow ample time for the file to download.
