Working to maintain clear communication between The Fluid Life Corporation Laboratories and our customers improves customer service and prevents delays in sample turn-around time ensuring delivery of results in a timely manner as part of the Fluid Life Quality Policy.

The Fluid Life Corporation urges all customers who currently use manual sample cards to consider the speed and convenience of using a paperless sampling system such as:

The following guidelines are to assist customers in completing the sample cards that are included with some Fluid Life sample kits. If you have any questions that are not answered here please contact your regional account manager for further detailed information, before submitting your samples to The Fluid Life Corporation.

Sample Card Completion Procedures


The following brochure gives essential information for completing sample cards. Please print it and share it with anyone in your company who does sample taking on equipment or anyone that submits samples to The Fluid Life Corporation for analysis.

Please fill out all sampling cards submitted with samples as completely as possible. If the information is complete, your sample will be processed in the shortest amount of time.

Sample cards that lack proper referencing information may cause testing to be delayed until the required information has been obtained from the customer.

Please identify sample points consistently.
If more than one employee is taking samples on the same component that information should be shared to prevent components and sample points from being improperly renamed or prevent changes in reference information for that component.

For first time samples taken from a component (testing unit) the following information is essential.

Please print the following information clearly on the sample card.

  • Complete name and address including contact information.
  • Email address is also helpful for reference and tracking(if available).
  • Unit Number--this number must remain consistent for subsequent samples.
  • Component type.
  • Serial number (if available).
  • Oil type including brand, grade and manufacturer.
  • The number of hours, months, kilometers or miles the oil has been used.
  • The total number of hours, months, kilometers, or miles the component has been used.
  • Was the oil changed? Yes or No?

Common Sample Card Errors:

  1. Unit number is often misnamed or renamed.
  2. Information essential for referencing the samples is not completed.

Necessary Referencing Information

Due to the volume of samples received at The Fluid Life Corporation, it is important for our customers to supply the following information to match and reference previous sampling information in our database. All samples must have at least the following 3 clearly printed on every sample card submitted for testing.
  1. Unit Number
  2. Component Type
  3. Site Location
  • Please supply the serial number of the component whenever possible. The serial number makes an excellent UNIT# because they are unique. If you choose to use the serial number as a UNIT#, it should be indicated in the UNIT# field.
After the card has been completely filled out please retain the carbon copy for your records and return the original copy with your sample

Training of your employees in the following areas will help to improve the quality of results needed for a successful lubrication management system.

  • Sample taking procedures.
  • Sample card completion procedures.
  • Sample submission procedures.

Other factors to consider.

  • Legal Survey Descriptions (LSD)s are often used as UNIT# for equipment in the oil and gas industry. This practice should be used with caution if your equipment moves from one location to another. If an LSD is used as the UNIT# and the location of the component changes please put the old LSD location on the card along with the new or contact Fluid Life to make the update.
  • Component field (ie transmission, engine, turbine etc.) is the primary field used to identify a sample point on a machine. If there is only one sample point on the component the location field can be left blank.
  • Location field (ie main, upper, lower, auxiliary, inline, reservoir, etc.) is designed to give more specific information if the component has more than one sample point. Like other sampling information, it is important that this information is consistent every time a sample is taken.
  • Fuel, Power Type, and Coolant fields are source information for the lab to perform appropriate tests and make appropriate recommendations where applicable.
