Ordering Sample Kits

Overview

Sample kits are typically sold in packages of ten. Each sample kit includes:

• Leak resistant white outer mailer bag with Fluid Life address.
• Sterile white test sample bottle with QR code on label.
• Sample card for sample information and customer contact information (if ordered).
• Testing and analysis of the sample itself.

Via Order Desk (Contract Orders)

If you currently have a contract with Fluid Life and/or are using a Purchase Order, you can use our online order form. You will be required to enter:

• The number of cases of bottles (10 per case)
• Detailed shipping / billing information
• Confirm your payment options
• Upload a PO (optional)
• Provide courier details if shipping via your approved courier,
• Any special instructions (i.e. rush order or specific delivery details)

Via Fluid Life Shop (Credit Card)

Prepaid industry test packages are avilable to order via credit card through our Fluid Life e-commerce shop.
